Psychiatric Rehabilitation Specialist
About The Position
Merakey is proud to provide services that put people first. We are currently seeking a Psychiatric Rehabilitation Worker to join our residential program in Colmar, PA, where staff work collaboratively to support growth, recovery, and daily living skills. The Psychiatric Rehabilitation Worker actively participates in individual assessments and development of individual service plan and goal setting with clinical liaison and supervisor. They educate the direct care staff on how to provide rehabilitation services that correlate to the goals identified on the service plan and how to appropriately document such services at our 10 Resident Community Residential Rehabilitation facility (CRR) in Colmar, PA.
Requirements
- Bachelor's degree in a human services field preferred.
- 1 year experience working in the Behavioral Health field/ population in a group home or a residential facility setting preferred.
- Must have a valid and clean driver’s license and clean driving record.
Nice To Haves
- Certified Psychiatric Rehabilitation Practitioner (CPRP) certification is a PLUS!
Responsibilities
- Clinical Assessment: Evaluate health concerns and conduct physical health assessments
- Medication Management: Dispense medications and monitor effectiveness and side effects.
- Care Collaboration: Work closely with the program Psychiatrist and interdisciplinary EAC team.
- Documentation: Complete accurate charting and clinical documentation using an EMR system.
- Health Education & Support: Provide health education and support recovery‑focused, holistic care.
