About The Position

The Psychiatrist serves as the clinical lead on the ACT (Assertive Community Treatment) Team at Conexio Care. This role provides psychiatric evaluation, diagnosis, medication management, and clinical oversight for adults with serious mental illness, co‑occurring disorders, and complex social needs. The Psychiatrist works closely with a multidisciplinary team to deliver recovery‑oriented, trauma‑informed community‑based care.

Requirements

  • Legally authorized psychiatric prescriber in Delaware.
  • Active licensure and required certifications.
  • Meet background check requirements.
  • Maintain compliance with HIPAA and regulatory standards.
  • Ability to work within a multidisciplinary ACT model.
  • MD or DO from an accredited medical school.
  • Completed psychiatric residency.
  • Board‑certified or board‑eligible in Psychiatry.
  • Active Delaware DEA registration.
  • Graduate degree in Advanced Practice Nursing.
  • Delaware‑licensed and certified Psychiatric Nurse Practitioner.
  • Authorized to diagnose and prescribe psychotropic medications.
  • Supervision by a Delaware‑licensed psychiatrist (2 hrs/week).
  • Minimum 5 years NP experience.
  • At least 2 years as a psychiatric prescriber in a multidisciplinary setting (preferred).
  • Experience in community behavioral health preferred.
  • Experience serving individuals with SPMI, substance use disorders, and co‑occurring needs.
  • Effective communication, clinical judgment, diagnostic competency.
  • Strong knowledge of psychiatric principles and best practices.
  • Ability to provide leadership and support to multidisciplinary teams.
  • Ability to manage crises and maintain professionalism.
  • Commitment to cultural humility and client rights.
  • Organizational and regulatory compliance skills.
  • Ability to use electronic health records and basic computer programs.
  • Valid driver’s license, personal vehicle, and minimum liability insurance.
  • CPR/first aid certifications and knowledge of medical terminology.
  • Ability to meet the physical demands of the job (lifting, driving, stair navigation).

Responsibilities

  • Conduct psychiatric assessments, diagnoses, and follow‑up appointments.
  • Prescribe and manage psychotropic medications, including ongoing reviews.
  • Deliver face‑to‑face clinical services (evaluations, therapy, home visits).
  • Provide crisis intervention, emergency response, and on‑call coverage.
  • Participate in involuntary medication hearings as needed.
  • Document all services and treatment plans in the electronic health record.
  • Utilize medication‑assisted treatment (MAT) where clinically appropriate.
  • Coordinate care with hospitals, primary care providers, and specialists.
  • Communicate directly with team members through meetings and consultations.
  • Model trauma‑informed, recovery‑oriented practices.
  • Support and guide ACT team members through consultation and education.
  • Participate in staff meetings, trainings, and professional development.
  • Ensure compliance with state, federal, and organizational regulations.
  • Maintain professionalism, accurate documentation, and good judgment.
