About The Position

We’re searching for a Psychosocial Coordinator to work with children and adolescents and their families in the Cancer and Hematology Center, someone who’s ready to be part of a growing team with amazing opportunities. In this position you will screen for the psychosocial risk of patients and/or families and to inform and/or develop risk intervention programs to support the families. Requirements

  • Bachelor's Degree Social Work, Child Life, Psychology, Kinesiology or other healthcare field required
  • Experience working with patients and families in a pediatric healthcare setting required

Responsibilities

  • Administers the psychosocial risk screening tool with patients and/or families.
  • Takes primary responsibility for the identification of eligible patients.
  • Works with the team to administer the screening tool with patients and families.
  • Coordinates and documents the results of the screening tool to help inform patient care and/or research initiatives.
  • Develops and maintains effective working relationships and communicates relevant information, in a timely manner, to the team.
  • Develops and prioritizes goals which support the psychosocial needs of patients and/or families as a whole and are congruent with the strategic plan.
  • Participates in patient and/or family programs to assist in meeting the psychosocial needs of patients and/or families.
  • Contributes to the efficient functioning of the department.
