About The Position

Jefferson County Public Health is seeking to hire a Public Health Nurse to join the Jeffco Family Connects team. The Family Connects Public Health Nurse provides home visits to any family caring for a newborn in Jefferson County. The goal is to assess family needs in several domains of physical and psychosocial well-being and facilitate referrals and connections to community agencies and the health care system for ongoing support and care. The Public Health Nurse will work with a team of clinical and non-clinical staff to implement this evidence-based Family Connects home visiting program. Therefore, the ability to communicate effectively with others is essential. Although it is desirable for applicants to have experience in maternal and child health, home visiting, lactation education and/or fluency in additional languages, all who qualify are encouraged to apply! This position will be expected to support the Jeffco Family Connects team to implement evidence-based strategies, leverage community actions or resources, and monitor progress for ongoing growth of the program Background on the Family Connects program: Family Connects provides universal evidence-based nurse home visits for postpartum families at around three weeks postpartum. During home visits, a registered nurse provides comprehensive assessment of the birthing parent and infant, tailored health education, and connection to community and medical resources as needed. As a data-driven population health model, Family Connects also helps communities identify gaps between community needs and community capacity to meet those needs. The goal of the program is to strengthen connections for families with newborns and link them directly to supportive community care resources. SCHEDULE: This full-time position has the potential to operate on our four-day work week (Monday-Thursday), or five-day work week (Monday-Friday). This position allows for some flexible work hours, upon approval from direct supervisor. This position is hybrid, with a large portion of the nurse’s work occurring in the community conducting home visits in addition to remote and in-office work as required. No mandatory weekend shifts! COMPENSATION: Hiring Range: $78,234.13-$86,926.81 Compensation will be determined based on education, experience and skills. Requirements

  • Bachelor’s degree in nursing and a minimum of one year of experience in public health nursing. An associate’s degree in nursing and 3 years of directly related experience may substitute.
  • Unrestricted registered nurse license in Colorado, or in a state that has enacted the RN Licensure Compact, and driver’s license.

  • Current CPR Certification at hire.

  • Any bilingual speaking ability is preferred but not required.

Responsibilities

  • Perform a health and physical assessment of the birthing parent and infant.
  • Apply knowledge and skills related to therapeutic communication, trauma-informed care, diverse family structures, social determinants of health, the Family Connects Model, postpartum health, infant health, infant feeding, lactation, parent-child relationships, safe sleep, intimate partner violence, perinatal mood disorders, infant crying, and substance use to identify and support family strengths and needs.
  • Assist parents in making appointments with health care providers, social services agencies, and other community referrals.
  • Serve as a liaison between families and providers/agencies as needed.
  • Utilize professional interpreters as needed when interacting with families with limited English proficiency.
  • Care for diverse populations with cultural humility.
  • Respond to client needs through flexible scheduling of home visits.
  • Apply effective time management, organization, and multi-tasking skills to manage a caseload of about 7 new families per week.
  • Maintain timely, accurate, and thorough documentation of interactions with families in compliance with program, JCPH and Board of Nursing guidelines.
  • Maintain licensure as a registered nurse, Basic Life Support certification, and valid driver’s license at all times.
  • Establish and nurture relationships, partnerships, and coalitions with community resources and services that provide support to families and participate in the community system of care for young children and their families.
  • Work collaboratively with the Family Connects team to identify resources, establish relationships with providers, maintain an updated list of resources, and identify gaps in needed community resources.
  • Attend and actively participate in case conferences, regular team meetings, clinical supervision, and other meetings as required.
  • Precept and mentor nursing students and new employees as requested.
