About The Position

The case manager for this position will work with individuals, families and nursing facilities to provide support for individuals who are PASRR Positive eligibility for mental illness (MI) specialized services and intellectual and developmental disability/related condition (IDD) specialized services. In addition, information will be gathered, and eligibility established for various LBHA and LIDDA services. Frequent travel to individual’s homes, community settings, nursing facilities and psychiatric hospitals in Collin County is required for this position. This position will require ability to work outside of normal business hours, weekends, and holidays to cover program needs. Other duties may be assigned to meet the needs of the program and individuals served. This case manager exemplifies service excellence by providing habilitation coordination for IDD PASRR Positive individuals and routine case management, skills training, medication training and support, crisis interventions and psychosocial rehabilitation services for MI PASRR Positive individuals with a trauma-informed and person-centered approach. Engagement with individuals served and exceptional customer service will be provided and documented in the electronic health record a minimum of 60 hours per month. At least 90% of IDD PASRR Positive individuals who have agreed to specialized services will receive habilitation coordination each month. Work quality and internal/external fidelity audits are expected to average at or above 90%. 95% or more of all PASRR evaluations are expected to be initiated within 7 days from the date a referral is received, and 95% or more of all evaluations are expected to be submitted within 7 days from the date the referral was received. This case manager practices good stewardship of resources by ensuring all required trainings are completed prior to due date and center-issued equipment is kept in good repair. Provides person directed habilitation coordination and case management to assist in navigating access to medical, social, vocational, financial, educational, and other individualized services. This case manager will follow up with at least 95% of individuals and/or their LAR who have refused MI PASRR specialized services at their 30/60/90 day follow ups and will follow up with at least 95% of individuals and/or their LAR who have refused habilitation coordination for IDD PASRR specialized services every 6 months to be offered a discussion about community living options as required by HHSC. Completing tasks in compliance with HHSC required timelines is expected. Integrity is necessary to ensure adherence to the Health and Human Services Commission, Texas Administrative Code, contracts, grants, and Lifepath Systems policies and procedures. Documentation which meets fidelity standards and documented into the electronic health record within 2 business days at a rate of 90% or above is expected. This case manager builds positive relationships with community-facing agencies and always maintains a professional relationship with all community stakeholders. Completing 95% of IDT/SPT meetings with nursing facilities within the HHSC required time frames is expected. Professional conduct with peers, leadership and support teams is expected at all times. Promoting choice and autonomy through a strength-based model is necessary to assist individuals served with accessing social determinants of health. For individuals who accept MI PASRR specialized services, a standard of 95% or higher of services are expected to be initiated within 20 business days after the IDT meeting with the nursing facility. Working independently and collaboratively to foster better outcomes for individuals served and community partners, to facilitate improvement in measurable ways, is central to LifePath Systems mission to serve. Ability to assess and de-escalate behavioral health crisis situations in calm and respectful manner. If Bilingual and receiving stipend, employee will be available to translate for other staff as needed and will utilize approved language in the provision of services as needed.

Requirements

  • Bachelor’s degree in psychology, social work, or related field from a four-year college or university.
  • Must have reliable transportation and a valid driver license.
  • Knowledge and experience in providing case management, skills training, and symptom management services to individuals with mental illness.
  • Ability to assess individual needs, to coordinate services, and to communicate effectively.
  • Knowledge of the social services system and the ability to connect individuals with appropriate services.
  • Ability to write clearly and professionally.
  • Ability to think clearly and logically to deal with potentially dangerous and/or emotional situations.

Nice To Haves

  • At least two years of experience in working with the community, preferred (i.e., social services, recovery, customer service, sales).
  • Bilingual (Spanish/English) preferred.
