As a Re-entry Navigator, you will play a pivotal role in empowering justice-involved individuals as they transition from Maricopa County jails back into the community, helping guide their reintegration by connecting them with vital services and resources to support their success in rebuilding their lives. This role is an essential part of the CHS Community Transitions Division, working closely with the Opioid Treatment Program (OTP). In this impactful position, you will screen patients for the OTP, provide education on medications that treat Opioid Use Disorder (OUD), facilitate connections to community clinics for ongoing support after release, and submit referrals to ensure patients receive the care they need.
