Job Description I. JOB SUMMARY The Record Integrity Analyst will facilitate accurate document and record integrity functions within the electronic health record (EHR) to support safe patient care. The analyst will monitor the appropriate work queues and systems to identify, resolve, and validate documents and data as needed for hospital and clinic encounters. The analyst will effectively communicate with a variety of stakeholders, including medical and clinical staff, patients, IT, and hospital leadership, to ensure resolution of documentation and data integrity issues. This position demonstrates expertise in the functions of HIM record completion and has extensive knowledge of regulatory standards and hospital policies that impact the EHR. The Record Integrity Analyst conducts concurrent and retrospective qualitative analysis and works cooperatively with medical staff to support a complete and accurate medical record. II. ESSENTIAL JOB RESPONSIBILITIES Support the Mission and Vision of LMH Health. Reviews, analyzes and ensures completeness of patient charts in compliance with hospital, state and regulatory standards. Assigns record deficiencies and manages provider record completion process. Prepares weekly documentation deficiency reports and sends to the appropriate Medical Staff providers. Researches declined or miss-assigned deficiencies, including orders and documentation to determine appropriate resolution. Keen attention to detail and accuracy are vital in this role, as is having a mindset of continuous performance improvement. Audits medical records and enters data into reporting system based on record requirements and provides feedback to medical staff on quality measures and proper documentation. Receives and communicates requested changes to EHR content via patient record amendment requests to medical staff. Performs quality control audits and error correction activities on scanned documents. Investigates and resolves record integrity issues in a timely manner. Performs other duties as needed or assigned. Regular and reliable attendance is an essential function of this position III. JOB QUALIFICATIONS Required: 2 years of relevant work experience in a related field with a HS diploma, No experience required with an Associates or Bachelor’s degree in Health Information Management or related field. Requirements
