About The Position

Under the supervision of the Recruitment Coordinator, this position assists in a variety of recruitment activities with the purpose of attracting and enrolling Adult Wellness Center participants, and Home Care Aide applicants/ candidates when needed. This position assists with community outreach and promotional activities associated with the agency’s older adult services programs. This position serves as a primary point of contact, providing information about the Adult Wellness Center, guiding individuals through the enrollment process, assisting with paperwork and verifying eligibility, and ensuring all necessary documentation is collected and processed. Provides ongoing support to address participant concerns, answer questions, and follow up on enrollment status.

  • High school diploma or GED required.
  • BILINGUAL IN SPANISH AND ENGLISH REQUIRED.
  • COMPUTER LITERACY IS REQUIRED. MUST BE PROFICIENT IN THE USE OF MICROSOFT OFFICE APPLICATIONS INCLUDING OUTLOOK, WORD, PUBLISHER, AND EXCEL, TO ENTER AND RETRIEVE DATA AS NEEDED.
  • HIGH SCHOOL-LEVEL MATH SKILLS ARE REQUIRED. WHEN NEEDED, ABILITY TO PREPARE STATISTICAL REPORTS REGARDING RECRUITMENT AND ENROLLMENT ACTIVITIES / OUTCOMES.

  • THIS POSITION REQUIRES THE ABILITY TO WORK A FLEXIBLE SCHEDULE AT TIMES, INCLUDING SOME SATURDAYS ON OCCASION.
  • MUST HAVE ACCESS TO A CAR (WITH VALID ILLINOIS DRIVER’S LICENSE AND INSURANCE) TO TRAVEL THROUGHOUT THE CITY AS PART OF COMMUNITY OUTREACH ACTIVITIES.

Responsibilities

  • Responsible for coordinating and participating in various community outreach activities and events (e.g. engagement with senior housing buildings, attending job and career fairs, senior health and wellness fairs/events, off-site presentations to community partners, churches, etc.)
  • May represent the agency’s older adult services programs in marketing events and other promotional activities.
  • Actively recruits prospective Adult Wellness Center participants and may assist in recruiting prospective home care aides when needed.
  • Responds to inquiries from individuals interested in attending the Adult Wellness Center or prospective home care aides (applicants) when needed.
  • Provides detailed information on adult wellness center and home care services when needed.
  • Assists in guiding individuals through the Adult Wellness Center enrollment process and home care aide application process when needed.
  • Engages in one-to-one consultations to assist individuals with paperwork, explaining options, and answering questions. When needed, provides community presentations regarding the older adult service programs.
  • Guides individuals through the process of enrolling in the Adult Wellness Center. Serves as the first point of contact, answering inquiries via phone, email, or in-person, and explains program details and other enrollment/eligibility information to help individuals make informed decisions.
  • Helps participants to coordinate initial referrals and assessment appointments with CCU/MCO. This process includes ensuring all necessary documentation is available.
  • Responsible for maintaining an ongoing and updated list of potential AWC applicants, including applicants’ contact information, as well as presentation and other engagement dates and follow-up activities. Responsible for following up with potential candidates and maintaining updated tracking information.
  • Promotes the AWC by coordinating and conducting on-site tours, attending various recruitment events, providing off-site presentations to community partners and targeted senior housing buildings (e.g. bingo presentations). Prepares materials to facilitate tours and presentations. Maintains a monthly task/activities calendar, under the direction of Supervisor and SS Director.
  • Coordinates with MCO or IDOA/CCU referral sources, to ensure all AWC-related enrollment data is accurately collected and submitted.
  • Responsible for building and maintaining an AWC referral resource list, by researching and contacting senior housing buildings, other social service agencies, various Case Coordination Units (CCUs), and various Managed Care Organizations (MCOs) representatives.
  • May assist in phone screening incoming résumés and job applications, in accordance with the HCA job requirements. Provides shortlists of qualified potential hires to Recruitment Coordinator.
  • Performs other duties as assigned by Recruitment Coordinator or SS Director.
  • Develops and maintains positive relationships with key, referring community partners.
  • IS INVOLVED AND KNOWLEDGEABLE OF THE AGENCY’S PERFORMANCE QUALITY IMPROVEMENT PROCESS AND SUPPORTS ALL PQI ACTIVITIES. INCORPORATES THE AGENCY MISSION, PHILOSOPHY, AND CORE VALUES INTO DAILY WORK AND DECISION-MAKING. ADHERES TO AGENCY POLICIES AND PROCEDURES.
  • PARTICIPATES IN REGULAR SUPERVISION WITH THE RECRUITMENT COORDINATOR. PARTICIPATES IN STAFF MEETINGS AND OTHER MEETINGS WHEN NECESSARY. ENGAGES IN PROFESSIONAL DEVELOPMENT TRAINING, AS NEEDED, TO ENHANCE PROFESSIONAL SKILLS. NURTURES POSITIVE AND COLLABORATIVE WORKING RELATIONSHIPS WITH OTHER AGENCY STAFF AND OUTSIDE PROVIDERS AND CULTIVATES A PROFESSIONAL AND RESPECTFUL WORK ENVIRONMENT, MODELING AND INITIATING RESOLUTION STRATEGIES, WHEN NEEDED.
  • SUBMITS PERSONAL TIME SHEETS AND ENSURES COMPLIANCE WITH AGENCY AND FUNDER GUIDELINES. ACCOUNTABLE FOR DAILY ACTIVITIES WITHIN THE ALLOWABLE ACTIVITIES APPROVED BY THE AGENCY. PROMPTLY REPORTS SITUATIONS THAT POSE A THREAT TO STAFF, PARTICIPANTS OR AGENCY, ACCORDING TO THE AGENCY’S INCIDENT / ACCIDENT REPORTING PROCEDURES AND IDOA CRITICAL EVENT REPORTING PROCEDURES
