About The Position

The Registered Nurse Assessment Coordinator (RNAC) provides for the initial assessment and periodic re-evaluation of all residents admitted to Skilled Nursing. Conduct Interdisciplinary Team Care Conference and implement and update resident Plan of Care. The Registered Nurse Assessment Coordinator (RNAC) would: Ensure accuracy of all sections of Multiple Data Set (MDS) coding to maximize company reimbursement consistent with the levels of treatment delivered and required for appropriate patient / resident care (CMI). Ensure accuracy of clinical paperwork (assessment and accompanying documentation) to maximize company reimbursement consistent with the levels of treatment delivered and required for appropriate patient / resident care. Nursing Educator: – Serve as a primary trainer to the nursing department to ensure documentation is accurate, timely, and thorough. This requires high levels of in-person interfacing with both residents and employees. Nurse Management: Serves as a key member of the nursing leadership team to clinically manage the unit in order to maximize patient-centered outcomes. This includes engagement in leadership standups and influence within interdisciplinary teams. Case management w/ managed care (maximize therapy and nursing services provided to residents in order to maximize patient-centered outcomes) Manage clinical and financial record keeping and dissemination of these records to appropriate payers. This includes overseeing UMR reviews and any appeals processes. Take joint ownership of care conferences with social workers Perform other duties as required. Requirements

  • Minimum 2 years related experience
  • Knowledge of state and federal regulations and requirements under Omnibus Budget Reconciliation Act (OBRA)
  • Graduate of accredited School of Nursing
  • Current license with the State of Pennsylvania for Registered Nurse
  • Must maintain current CPR Certification

Nice To Haves

  • Registered Nurse – licensed in State of Pennsylvania not required but highly desired
  • AANAC certification (desired, must have within 12 months of employment)
