Sentara is hiring a Registered Nurse Case Manager II for the Specialty Pharmacy at Norfolk General Hospital in Norfolk, VA. Location: Sentara Norfolk General Hospital, Norfolk, VA Status: Full Time (40 hrs/wk) Hours: Monday-Friday 8am-5pm (will be required to work until 6 pm two days per month on a rotating basis) No Weekends The RN Case Management Pharmacy Case Manager II coordinates the care and services of selected Medicaid health plan member populations across the continuum of care, promotes effective utilization and monitoring of health care resources, and assumes a collaborative role with all members of the healthcare team to achieve optimal clinical and resource outcomes. RN Pharmacy Case Manager II must be able to effectively assess and coordinate the member’s needs; maximize their available benefits; assist in creating a cost-effective plan of care; and identify high-risk and high-utilization cases. Assigned cases may include components of medical, behavioral health, disease management, complex case management, and/or concurrent (Utilization Review) review. Sentara Norfolk General Hospital , located in Norfolk, VA, is a 525-bed tertiary care facility that is home to the only Level I Adult Trauma Center and burn trauma unit in Hampton Roads, and also serves as the primary teaching hospital for Eastern Virginia Medical School. In addition to the high-quality heart program at Sentara Heart Hospital, our facility is home to Nightingale Regional Air Ambulance and several other dedicated facilities and specialized services.
