About The Position

We are seeking a Registered Nurse Case Manager to support the Maine Army National Guard (MEARNG) in Augusta, ME. In this role, you will support Soldier medical readiness by coordinating non-clinical case management activities, readiness event follow-up, and medical records tracking to help MEARNG maintain Individual Medical Readiness (IMR). Requirements

  • Must be a Registered Nurse III (RN III)
  • Must have at least 2 years of experience in case management
  • Must have at least 3 years of clinical experience
  • High school diploma or equivalent required

Nice To Haves

  • A Certified Case Manager (CCM) certification is preferred
  • Undergraduate degree preferred

Responsibilities

  • Support Soldier Medical Readiness
  • Review privacy rights with Service Members and secure signed Release of Information (ROI) forms
  • Interview Soldiers by phone or in person and document interactions in case tracking systems
  • Track each Soldier's medical readiness status and maintain accurate case files
  • Educate Soldiers on resources that improve medical and dental readiness
  • Manage Medical Appointments
  • Audit open cases every 21 days to confirm follow-up and communication remain current
  • Escalate flagged (P3/P4) medical profiles to the Deputy State Surgeon (DSS) and track evaluation actions
  • Schedule required medical and audiology appointments for assigned Soldiers at designated health facility
  • Coordinate treatment plans and follow-up care with medical providers and facilities
  • Confirm appointment attendance with the Soldier’s unit and obtain required follow-up documentation
  • Manage Soldier care from initial identification through return-to-duty or administrative separation
  • Support Medical Readiness Events (MRE)
  • Review readiness systems before each Medical Readiness Event (MRE) to flag non-deployable Soldiers
  • Support MRE and Soldier Readiness Processing (SRP) events by providing required services
  • Check post-event referral lists daily and schedule follow-up visits with Army medical providers
  • Update readiness records with follow-up information in the Medical Operational Data System (MODS)
  • Manage Medical Records
  • Scan and index medical documents and route records to the Medical Command (MEDCOM) custodian
  • Enter make-up service vouchers in the QTC Reserve Health Readiness Program when Soldiers miss MRE/SRP events
  • Assemble Line of Duty (LOD) investigation packets in coordination with Health Service Support
  • Complete DD Form 771 and submit Soldier eyewear orders in the Spectacle Request Transmission System (SRTS)
  • Mail medical records to and from mobilization sites at least 30 days before each Soldier’s report date
  • Comply with Individual Medical Readiness (IMR) and HIPAA requirements (e.g., annual HIPAA training, etc.)
