About The Position

Constellation Health Servicesis seeking a Registered Nurse to join our start-up and survival team (S&S)!! This role is for the person who is comfortable with being a road warriorOne who can walk into a program that needs a lot of attention and enjoys being that lifeline, can multitask and stretch where most are unable to. Requirements

  • MUST have hospice experience in facilities and the community at minimum 2 years.
  • Current and active RN license in the state of employment.
  • In-depth understanding of end-of-life care principles and practices and certified home health.
  • Exceptional communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Compassionate and empathetic approach to patient care.
  • Basic knowledge of iOs operating system.

Nice To Haves

  • Bachelor’s degree in nursing (BSN) preferred.

Responsibilities

  • Delivery of all aspects within their scope of practice, RN CM and On Call (7 on & 7 Off), where needed, in all environments that a patient calls home (skilled nursing facility, home, etc)
  • Deliver compassionate and comprehensive nursing care to patients facing end-of-life challenges.
  • Provides skilled nursing care including administration of medications and treatments as prescribed in the physician’s plan of care for patients in their home setting.
  • Conduct thorough assessments to identify patient needs and develop individualized care plans.
  • Manage medications and treatments as prescribed by the physician.
  • Provide emotional support and guidance to patients and their families.
  • Educate patients and families about hospice care and end-of-life issues.
  • Collaborate effectively with interdisciplinary team members to ensure coordinated care.
  • Maintain accurate and up-to-date patient records.
  • Ability to travel up to 100% of the time.
  • On call rotation to include 7&7 in addition to their personal case management duties to support the site.
