About The Position

Hill Country Community Clinic is a mission driven Federally Qualified Health Center (FQHC) and Certified Community Behavioral Health Clinic (CCBHC) located in beautiful rural Northern California. We serve diverse and underserved communities across Shasta County and surrounding areas through integrated primary care, behavioral health, and substance use disorder treatment services. POSITION SUMMARY: Under limited supervision, assess client health needs and develop care plans in a holistic manner; triage and administer appropriate medical and/or behavioral health care to all clients by protocol; review medical records and provide case management; assist upper management with quality assurance, planning and implementing strategic goals; develop and implement client programs.

Requirements

  • Current BRN licensure in California.
  • Must have at least 1 year experience in clinical nursing.
  • Must have knowledge of standard practices and processes of RN’s, LVN’s, MA’s and other nursing personnel.
  • Must have strong leadership skills.
  • California Registered Nurse license.
  • Must have current CPR certification.
  • Current drivers’ license
  • Must have intermediate level language skills. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before individuals, groups or employees of organization.
  • Must have basic math skills. Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentages.
  • Must have very high level reasoning skills. Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical information and deal with several abstract and concrete variables.
  • To perform this job successfully, an individual should have knowledge basic working knowledge of Windows operating systems, Microsoft Office Suite - Word, Excel, Outlook, and Internet browsers.
  • Must have working experience with medical management software such as Health Pro and Medi-Tracks.
  • Must have good working knowledge of standard nursing equipment.

Nice To Haves

  • Experience in rural community clinic preferred.

Responsibilities

  • Authorizes medication refills requests (according to protocol).
  • Oversees dispensary services, outdates of medications and supplies, and conducts monthly crash cart checks and inventory, act as pharmacy consultant liaison, ordering of meds.
  • Triages and cares for clients as needed, both walk-in and over the telephone, including scheduling appointments as needed, consulting with practitioners and inform primary care provider of advice/instructions given and actions taken.
  • Manage complicated cases (patient advocate, interface with provider, in-house referral, controlled substance management).
  • Supports medical teams in any capacity, delegating tasks to other staff based on scope of practice, training and competence.
  • Teach and counsel clients regarding health and wellness issues, lab results, imaging results, diabetes and diet.
  • Record mammogram and PTINR results and notify clients per protocol.
  • Attend all meetings/trainings/conferences as appropriate for position, including weekly HCCC meetings.
  • Manage desktop in a timely manner, assist providers and staff including medication refills as needed.
  • Follow-up on Partnership ER notifications, mail condolence cards, follow-up on all after-hours calls.
  • Other duties as assigned by Administration.
