About The Position

Hospital Sisters Health System (HSHS) is seeking a Registered Nures-Home Health to join our team. The Registered Nurse-Home Health, in conjunction with the Case Manager, utilizes physicians' orders and professional skills to develop and implement a home health plan that meets each patient's specific needs and is in compliance with accrediting organization standards, federal and state regulations, reimbursement guidelines, as well as agency policies and procedures. The Registered Nurse-Home Health is responsible for planning, coordinating and documenting care for an assigned caseload of patients. Participates in Quality Assessment and Performance Improvement (QAPI) activities. Position Specifics: o Department: Home Health o Core Function: Nursing o Schedule: Day Shift, 40 hrs/wk o Facility: St. Vincent's Hospital and surrounding area o Location: Green Bay, WI o Compensation that aligns with your experience

Requirements

  • Graduate from an accredited and approved school of nursing accepted for licensure in the state of practice is required.
  • Valid drivers license, automobile insurance and own transportation to make home visits is required.
  • Licensed as a Registered Nurse (RN) in the state of practice is required.
  • Wisconsin: RNs may practice with a valid multi-state RN licenses as outlined in the Nurse Licensure Compact rules.
  • Basic Life Support (BLS) is required.

Nice To Haves

  • Bachelor's Degree in nursing is preferred.
  • Prior experience in home health or hospice is preferred.
  • 1 year acute nursing experience is preferred.
