Key responsibilities include: Conduct comprehensive assessments of patients and families to identify physical, psychosocial, and environmental needs Develop, implement, and manage individualized plans of care in collaboration with physicians and the interdisciplinary team Perform initial evaluations and ongoing reassessments to monitor patient progress Provide skilled nursing care and education to patients and caregivers Communicate significant findings, changes in condition, and safety concerns with physicians, clinical leadership, and care team members Update and revise care plans based on patient needs and clinical outcomes Maintain accurate and timely documentation in accordance with regulatory and organizational standards Ensure strict compliance with HIPAA privacy regulations and patient confidentiality Travel within the service area to provide care in patients’ homes What We’re Looking For Current, unrestricted Registered Nurse (RN) license in the state of practice 1+ year of clinical nursing experience Ability and willingness to travel within the assigned coverage area Ability to lift, push, pull, or support up to 50 pounds, including assisting with patient positioning or transfers Valid driver’s license, reliable vehicle, and current auto insurance per state requirements Associate or Bachelor’s Degree in Nursing (BSN preferred) Home Health or Hospice experience preferred Experience with HomeCare HomeBase (HCHB) preferred Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs. ✨ If you’re looking for a nursing career where you can truly connect with patients, practice with independence, and make a meaningful difference every day, we encourage you to apply today and join the Elara Caring team.
