About The Position

This is a weekend + 1 position. It is every Saturday and Sunday 8am-8pm and every Thursday 3pm-11pm. The Registered Nurse (RN) Coordinator handles the day-to-day coordination of the team clinical functions, assists in implementing the patient plan of care with staff, volunteers, and outside agencies. Communicates as a liaison between staff, patients, and outside agencies. Assists with maintaining records. May involve direct patient care.

Requirements

  • Graduate of an accredited School of Registered Professional Nursing with a current RN licensure in the State of New York.
  • Valid Driver's License, free of serious violations, and vehicle required for use when on duty.
  • 1-2 years of nursing practice as a Registered Professional Nurse preferred.

Responsibilities

  • Assists in coordination and implementation of the patient plan of care.
  • Handles phone calls from the patient, primary care giver, or designee regarding any questions or concerns.
  • Collaborates with the interdisciplinary teams, RNs, or Team Manager.
  • Coordinates and schedules patient visits.
  • Provide timely documentation/data entry per Hospice time frames and deadlines.
  • Communicates current health status and needs of patients to appropriate team professionals.
  • Assists the team and Team Manager with other administrative functions, as necessary.
  • May occasionally be called on to make field visits
  • May periodically perform the basic Charge RN functions when needed to cover the designated Charge RN
