About The Position

Position Purpose: Registered Nurse (RN) coordinates the total plan of care for the patient in the home care setting assessing, planning, implementing and evaluating.

Requirements

  • Graduate of a nursing school program
  • AZ RN License, or compact state RN License
  • Level One Fingerprint Clearance Card, or application, required within 20 days of hire
  • AZ Driver's License
  • BLS Certification from American Heart Association or American Red Cross
  • 1-2 years acute care nursing or home health experience
  • Requires travel throughout local service area and interactions with customers in their home setting.
  • Duties have routine potential for exposure to blood, body fluids or tissues; ability to move patients >300lbs; sit, stand, walk, and bend duration of work shift.
  • Ability to address multiple priorities simultaneously, possess good oral and written communication skills, knowledgeable with business office equipment.

Responsibilities

  • The Home Health RN coordinates the implementation of a nursing plan of care for each assigned patient.
  • Works under the direction of the physician and the Arizona State Nurse Practice Act to provide nursing care to patients in compliance with nursing standards and regulations.
  • Utilizes the nursing process to make continuing assessments of the patient’s total health needs; implements nursing interventions based on assessment data; develops and implements the nursing plan of care and evaluates the patient’s response.
  • Collaborates with healthcare providers, nursing staff, other patient caregivers, case managers, and health information team members to improve the quality and completeness of documentation of care.
  • Adheres to behavioral expectations and policies; maintains patient and staff confidentiality; supports a healthy work environment through skilled communication, true collaboration, effective decision making, and meaningful recognition.
