Registered Nurse (RN) MDS Coordinator - Part Time
About The Position
MDS Coordinator, RN – DAY Shift Seymour, WI (15–20 minutes from Green Bay and Appleton) Pay Based on Experience Current Openings (Monday - Friday) 30 hours per 2-week pay period Compassionate care. Position Summary The MDS Coordinator, RN, plans, coordinates, supervises, and implements nursing services for residents as part of the geriatric interdisciplinary team. This role ensures accurate completion of MDS and Medicare documentation and assessment forms in compliance with state and federal regulations for long-term care. The MDS Coordinator makes clinical decisions to guide effective nursing practice, assists in direct resident care as needed, and coordinates the assessment, care planning, and interdisciplinary team communication for assigned residents.
Requirements
- Wisconsin Licensed Registered Nurse (RN)
- Minimum 3 years of nursing experience, with recent geriatric nursing experience
- At least 1 year of MDS experience required
- Must demonstrate physical ability to spend 50% of working hours standing, moving, lifting, and assisting residents as needed (up to 50 lbs.)
- Must be able to respond to emergencies and support resident evacuation
- Strong communication skills in English; ability to work under mental and emotional stress

Responsibilities
- Assess resident health status and communicate with the healthcare team
- Complete and coordinate MDS and Medicare assessments according to regulations
- Lead or delegate care conferences; maintain liaison with residents, families, and departments
- Supervise and direct team members in delivering care per facility policies
- Prepare and administer medications and treatments as ordered
- Maintain resident confidentiality, privacy, and safety
- Participate in staff development, orientation, and professional learning activities
- Work beyond normal hours when necessary, including weekends and holidays
