Requirements
- Active RN license in the state of Pennsylvania
- CPR certification (BLS required; ACLS a plus)
- Experience in home health, medical-surgical, or geriatric nursing is a plus
- Strong communication and critical-thinking skills
- Compassionate, detail-oriented, and adaptable in various care environments
- 1-2 years' experience with Vent & Trace with ALS
Nice To Haves
- Experience in home health, medical-surgical, or geriatric nursing is a plus

Responsibilities
- Clinical Nursing Care
- Conduct full nursing assessments
- Administer medications and treatments as prescribed
- Provide skilled nursing interventions such as wound care, IV therapy, catheter care, and monitoring of medical equipment
- Monitor and document changes in patient condition and communicate findings with physicians and family
- Care Coordination & Leadership
- Collaborate with LPNs, CNAs, and other care team members to ensure comprehensive care
- Supervise and guide paraprofessional staff as needed
- Ensure proper documentation in compliance with regulatory and agency standards
- Patient & Family Support
- Educate patients and families on disease processes, medications, and ongoing care needs
- Provide emotional support and guidance during challenging health transitions
- Promote patient safety and dignity in all aspects of care
