Registered Nurse (RN) HSHS Home Care
About The Position
The Registered Nurse-Home Health, in conjunction with the Case Manager, utilizes physicians' orders and professional skills to develop and implement a home health plan that meets each patient's specific needs and is in compliance with accrediting organization standards, federal and state regulations, reimbursement guidelines, as well as agency policies and procedures. The Registered Nurse-Home Health is responsible for planning, coordinating and documenting care for an assigned caseload of patients. Participates in Quality Assessment and Performance Improvement (QAPI) activities. Position Specifics: Department: HSHS Home Care Core Function: Nursing Schedule: Full-Time, 40 hours/week (position includes on-call rotation) | On-call rotation 1 week at a time, typically every 6th week Facility: St. Anthony’s Hospital Location: O'Fallon, IL Service Area
Requirements
- Graduate from an accredited and approved school of nursing accepted for licensure in the state of practice is required.
- Licensed as a Registered Nurse (RN) in the state of practice is required.
- Basic Life Support (BLS) is required.
- Valid drivers license, automobile insurance and own transportation to make home visits is required.
Nice To Haves
- Bachelor's Degree in nursing is preferred.
- Prior experience in home health or hospice is preferred.
- 1 year acute nursing experience is preferred.
