About The Position

Colorado PACE has an immediate opening for a Rehab Aide PACE. Location: Aurora, CO Status: Full-time Schedule: Monday-Friday, 8:00a - 4:30p Hourly Pay Range: $19.26 - $20.00 Supplemental Pay: Based on position, schedule and/or availability: Paid Mileage/Overtime Mileage and Expense Reimbursement: Sixty-two and a half cents per mile – one of the highest in the industry! REWARDING WORK YOU WILL DO: The Rehab Aide provides essential support to Physical, Occupational, and Speech Therapists. You will assist in the delivery of rehabilitative plans, manage specialized equipment, and ensure that the therapy gym is safe, welcoming environment for our participants. This role requires a blend of physical stamina, technical organization, and a deep empathy for the geriatric population.

Requirements

  • High school diploma or equivalent.
  • Minimum one years' experience working with the frail and elderly poplulation, home care, or geriatric experience.
  • Valid BLS Certification Required.
  • Valid Colorado driver’s license and proof of auto insurance.
  • Ability to lift/push/pull minimum of 35 lbs.

Nice To Haves

  • Associate degree in health related field preferred.
  • CNA or Restorative Aide certification is a plus.

Responsibilities

  • Assists therapists with gait training, transfers, and therapeutic exercises as directed by a licensed PT or OT.
  • Utilizes interpersonal skills that promote interdisciplinary collaboration and communication.
  • Leads or supervises individualized exercise programs and range-of-motion (ROM) activities once the therapist has established the protocol.
  • Safely escorts participants to and from the therapy clinic, ensuring their comfort and safety.
  • Provides feedback to the therapy team regarding a participant’s progress or changes in mood/physical status.
  • Coordinates therapy appointments with the transportation and activities departments to ensure maximum participation.
  • Seeks opportunities for professional growth, including demonstrating the ability to accept direction from professional staff of all disciplines and to receive constructive performance feedback.
  • Attends a minimum of three hours of continuing education programs per calendar quarter.
  • Participates in the orientation of new members of the IDT and students.
  • Assists in the fitting of walkers, canes, and wheelchairs.
  • Perform minor repairs and adjustments to durable medical equipment (DME).
  • Tracks all equipment loaned to participants’ homes; manage the cleaning and sanitation of gym equipment according to infection control standards.
  • Accurately records participant attendance and "minutes" for non-skilled activities in the Electronic Health Record (EHR).
  • Establishes therapeutic relationships with patients and families from a variety of cultural, ethnic and socioeconomic backgrounds.
  • Provides emotional support to patients and families in a manner that fosters independence and affirms existing strengths.
  • Reports to the therapy team on a regular basis and whenever changes in patient condition warrant.
  • Articulates and utilizes universal precautions and use of specialized personal protective gear as appropriate to patient diagnosis.
  • Maintains competencies to perform participant care tasks.
  • Cleans durable medical equipment as necessary.
  • Documents care provided and the response of the patient/family per agency policy.
  • Participates in departmental and/or agency-wide quality improvement activities.
  • Participates in performance improvement activities.
