Learning Services, a Collage Rehabilitation Partner, is a growing organization dedicated to excellence, honesty, and ethics in the treatment of our clients and each other. For more than 35 years, we’ve developed programs to meet the diverse needs of clients with moderate to severe traumatic and acquired brain injuries. We provide treatment for post-concussion syndrome, stroke, memory loss, neurobehavioral issues and accident or sports-related head injuries.

Requirements

  • A bachelor’s degree along with case management and health care industry experience is required
  • A valid driver's license, insurance and responsible driving behaviors must be maintained
  • Strong clinical skills as shown by the ability to manage crisis situations, assess clients' needs and help formulate, prioritize and monitor treatment plans is necessary

Nice To Haves

  • A master’s degree and CRC or CCM designation is preferred

Responsibilities

  • Responsible for all case provisions for the active rehabilitation caseload, for both the residential neurorehabilitation and day treatment clients.
  • Leads team meetings and interfaces with the treatment team to establish rehabilitation goals and coordinates treatment necessary to support the client’s treatment plan. Assists the team on goals and the discharge plan.
  • Responsible for ensuring the implementation of the clients’ treatment plan(s), from admission through discharge and follow-up
  • In consultation with the clinical team, identifies, collaborates, and consults with physicians and other health care providers outside of the primary treatment team.
  • Coordinates schedules, therapies, and other appointments/services
  • Responsible for regular contact with families and funders
  • Ongoing internal report writing, clinical documentation, and chart documentation
  • Facilitating funding issues as needed.
  • Serves as client advocate by communicating clients’ needs and issues to team members and other designated individuals, including a plan for resolution and follow-through as indicated.
  • Support the Program Director in program case managers’, onboard training, and performance to ensure case management standards of performance meet expectations.
