About The Position

Time-Limited Subsidies (TLS) are a critical component of Los Angeles County’s Continuum of Care permanent housing system. This program operates under a Housing First, low-barrier, Harm Reduction, and Progressive Assistance model to support homeless individuals and families with complex needs in obtaining and stabilizing permanent housing. Catholic Charities of Los Angeles, Inc. is seeking a high-performing Release Services Case Manager with demonstrated experience providing intensive, high-touch case management to individuals and families impacted by substance use disorders and mental health conditions. This role requires advanced problem-solving skills, clinical awareness, and crisis intervention capacity to support housing stability.

Requirements

  • Bachelor’s degree in social work, psychology, human services, or a related field required; Master’s degree preferred.
  • Minimum 3–5 years of experience providing intensive case management in homeless services, housing programs, or behavioral health settings.
  • Required experience working with individuals experiencing substance use disorders, including familiarity with Harm Reduction, relapse prevention, and engagement strategies.
  • Required working knowledge of mental health disorders and their functional impact on housing stability.
  • Demonstrated ability to provide high-touch, problem-solving-oriented case management in high-acuity environments.
  • Experience working with diverse populations, including individuals and families from African American, Latino, Asian, and other communities.
  • Strong crisis intervention, de-escalation, and conflict-resolution skills.
  • Knowledge of Housing First, Progressive Assistance, and Continuum of Care principles.
  • Excellent documentation, organization, and time-management skills.
  • Independent judgment, accountability, and initiative.
  • Valid California Driver’s License (Class C), auto insurance, and willingness to travel.
  • Ability to pass a drug screen and DOJ/FBI background check.

Responsibilities

  • Provide intensive, field-based and office-based case management to homeless individuals and families enrolled in the Time-Limited Subsidies program.
  • Apply substance use disorder (SUD) service models and harm reduction principles to support participant engagement and housing retention.
  • Demonstrate working knowledge of common mental health disorders (e.g., depression, anxiety, PTSD, bipolar disorder) and how they impact housing stability.
  • Conduct ongoing risk assessments, safety planning, and crisis intervention related to behavioral health and housing instability.
  • Utilize problem-solving and motivational engagement techniques to address barriers such as relapse risk, disengagement, non-compliance with lease terms, or crisis episodes.
  • Develop, implement, and routinely update individualized housing stabilization and retention plans.
  • Adjust service intensity using Progressive Assistance based on participant acuity and risk level.
  • Coordinate referrals and follow-up with behavioral health providers, substance use treatment programs, and community-based supports.
  • Provide budgeting education, landlord mediation, lease compliance support, and appointment coordination.
  • Maintain timely, accurate, and compliant HMIS and case file documentation in alignment with contract and funder requirements.
  • Participate in case conferences, clinical consultations, and supervision.
