About The Position

A leading managed care organization serving New Hampshire Medicaid members is seeking an experienced and detail-oriented nurse to support long-term care and rehabilitation admissions throughout the state. This fully remote position focuses on coordinating placements into skilled nursing facilities and extended care settings while ensuring timely, appropriate transitions of care. This role is ideal for an RN or LPN with experience in long-term care, home health, hospital discharge planning, or post-acute admissions who understands New Hampshire Medicaid, long-term care eligibility, and level-of-care criteria.

Requirements

  • Active, unrestricted New Hampshire RN or LPN license required
  • Minimum 2–3 years of experience in long-term care, home health, rehabilitation, or hospital admissions
  • Working knowledge of New Hampshire Medicaid long-term care eligibility and level-of-care criteria
  • Strong communication, organization, and documentation skills
  • Ability to work independently in a remote environment
  • Proficiency with electronic medical records and care coordination platforms

Nice To Haves

  • Managed care or health plan experience
  • Utilization management or case management experience
  • Experience with hospital discharge planning or SNF placement
  • Experience working with New Hampshire Medicaid waiver programs or long-term services and supports (LTSS)
  • Bilingual English and Spanish a plus

Responsibilities

  • Coordinate admissions into long-term care, skilled nursing facilities, and rehabilitation programs across New Hampshire
  • Review clinical documentation to determine appropriate level of care and placement eligibility
  • Facilitate transitions from hospitals to SNFs or long-term care settings
  • Serve as liaison between the health plan, facility admissions teams, and hospital discharge planners
  • Manage prior authorizations and eligibility verification within scope of licensure
  • Collaborate with utilization management, case management, and care coordination teams
  • Educate members and families regarding Medicaid benefits and care transitions
  • Maintain accurate documentation in EMR and care coordination systems
  • Ensure compliance with New Hampshire Medicaid guidelines and long-term care regulations
