Requirements

• Registered Nurse (RN) or Registered Practical Nurse (RPN) diploma with five (5) years of Nursing experience
• Healthcare and case management experience preferred
• Bilingualism (French and English) strongly preferred
• Experience in oncology is an asset
• Strong patient focus and critical thinking skills
• Effective communication skills, both orally and in writing
• Ability to multi-task and work under pressure
• Proficiency in Microsoft Office programs

Nice To Haves

• Healthcare and case management experience preferred
• Bilingualism (French and English) strongly preferred
• Experience in oncology is an asset

Responsibilities

• Responsible for assigned caseload and meeting inbound/outbound call targets
• Manage reimbursement and appointment coordination for patients
• Provide clinical support and guidance on the enrolment process
• Review feasibility of requests for medication administration
• Ensure accurate and up to date documentation for patient appointments
• Lead disease-state training of new/existing team members
• Provide feedback on protocols and inter-departmental relationships
• Report all Adverse Events to the assigned units/departments
