What You’ll Do As a Resident Care Manager, you’ll play a key leadership role in ensuring residents receive exceptional care while supporting and guiding the clinical team. Responsibilities include: Coordinating and overseeing daily nursing care in collaboration with physicians and the interdisciplinary team Leading and supporting licensed nurses and CNAs through coaching, guidance, and daily oversight Assisting with admissions, transfers, and discharges while ensuring accurate documentation Completing and managing MDS assessments and related clinical documentation Conducting care conferences and collaborating with residents and families to create individualized care plans Monitoring resident conditions and ensuring care plans are updated and followed appropriately Promoting a positive and supportive environment for residents and staff What We’re Looking For Current Registered Nurse (RN) or (LPN) license required Strong leadership and team-building skills Experience in skilled nursing or long-term care preferred Knowledge of care planning, MDS, and regulatory compliance Passion for providing high-quality resident-centered care Why Join Pine Ridge Post Acute?
