We are looking for a Case Manager to enhance the quality of our client management, maximize satisfaction and promote cost effectiveness. The successful candidate will be accountable for the full patient care cycle. Case Manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human services needed. The Residential Case Manager will coordinate client services per treatment plan for the adult clientele. The case manager will be assigned specific clients at time of intake and will be required to program and monitor such things as court appearances, doctor appointments, family therapy sessions, and discharge referrals. The Residential Case Manager will also maintain open communication and/or meet with family, courts, and others as necessary. The Residential Case Manager will meet with members of the Treatment Team.
