About The Position

Purpose: To integrate, coordinate and monitor each person’s residential and comhab-R support programs and plans with a person centered approach.

Requirements

  • At least one year of experience working in the field of developmental disabilities and is at least one of the following: a Doctor of medicine or osteopathy, a Registered Nurse, or a person who holds at least a Bachelor’s degree in a professional category specified as: Occupational Therapy, Physical Therapy, Social Work, Speech-Language Pathology or Audiology, Recreation Services (including dance, art, music, or physical education), Dietetic Services, Human Services (including psychology, sociology, rehab counseling, special education).
  • Flexibility of schedule to meet job duties/individual needs.
  • SCIP-R Theory.
  • Ability to work independently and reliability in decision making.
  • Proficiency in Microsoft Word and Excel.
  • Demonstrated ability to communicate effectively in oral and written form. Excellent verbal, written and presentation skills.
  • Strong organizational skills with a keen ability to prioritize and multi-task. Detail oriented.
  • Well developed team skills, unquestioned integrity, and the experience, confidence and presence to effectively handle interpersonal relationships and deal with sensitive issues.
  • Valid NYS Driver’s License acceptable to agency insurance standards, preferred. Maintain valid insurance and provide insurance card if you use your own vehicle.
  • Adhere to Agency policies including attendance
  • Interaction with persons receiving services may include SCIP, bending, reaching, pushing and/or pulling.
  • Effective verbal and written communication skills. A demonstrated interest and ability to address the personal outcomes and needs of individuals served.
  • An ability to work independently and to facilitate group discussion(s).
  • Organizational skills, reliability in decision making.
  • Ability to maintain a high level of professionalism and confidentiality. Ability to remain calm in stressful or demanding situations.
  • Sitting and/or computer use for extended periods of time.
  • Ability to perform multiple tasks.

Responsibilities

  • To develop, through a person centered approach, and document each person’s initial and on-going individualized plan of protective oversight (IPOP). The IPOP will identify the individual’s strengths, areas of improvement and any behavioral support needs. Collaborate with person served and interdisciplinary team members to identify the person’s needs and valued outcomes. Update the IPOP as needed when assessments are updated.
  • To monitor individuals ensuring each individual is seen a minimum of one time a month. Reviews data and progress and revises Residential Staff Action Plans (SAP) based on individual need and performance. A QIDP must be responsive to the individual’s unique needs and there must be a foundation of professional knowledge that can assist with development of the SAP.
  • To be able to demonstrate to Direct Support Professional how to work on a specific goal outlined in the SAP and offer support as needed. The QIDP will demonstrate methods, who is responsible for the program implementation and the type of data and frequency collection.
  • To ensure service design and delivery by observation at the home to provide an individual with an appropriate person centered program based on their unique wants, dreams, and needs.
  • To ensure consistency among external and internal programs and disciplines.
  • To ensure adequate environmental supports and assistive devices are present to promote independence.
  • To ensure that any discrepancies or conflicts between programmatic, medical, dietary, and vocational aspects of the individual assessment and programs are resolved
  • Provide training session(s) to direct support staff as needed and as appropriate.
  • To follow up on recommendations for services, equipment or programs.
  • To ensure that adequate environmental supports and assistive devices are present to promote independence.
  • Other clinical duties as assigned.
  • Mentor Residential managers on the role of QIDP and monthly summaries
  • Participate as a member of Agency committees as assigned, as well as attending trainings, such as CQL
  • Any and all other related duties as assigned by supervisor.
