About The Position

Why M Physicians? The Pediatric Explorer Clinic provides innovative and comprehensive care to children of all ages. We focus on the whole-person health of each of our patients with our integrated approach to care and treatment. We provide a variety of services that are specifically tailored to the needs of infants, children, teens, and young adults, including NICU follow-up care, medication therapy management, and physical medicine and rehabilitation. In addition, we offer a variety of medical specialties, including pediatric, heart care, neurology and neurosurgery, rheumatology, and genetics and metabolism. Our multidisciplinary team works together to deliver the best care possible in an environment that is welcoming for children from infancy to adolescence. What you will do as an RN Care Coordinator: Coordinates care across the health care continuum within designated sub-specialty population(s) Serves as a single point of contact by facilitating and coordinating transitions in care within and between care settings, providers, and services Completes pre-visit planning and post-visit wrap-up and determines vital follow-up appointments and/or treatments to optimize patient visits Facilitates proactive disease management and preventative health care using evidence-based practice Rooms patients for the surgeon(s) as needed, complete clinic readiness including records review and provides administrative support as needed. What you will need: Current Minnesota RN license 3+ years of Clinical Nursing experience BLS certification Demonstrated experience in patient advocacy and education, coaching, and proactive health care management along with EMR/digital literacy Location: Pediatric Explorer Clinic, 606 24th Ave S. Requirements

  • Current Minnesota RN license
  • 3+ years of Clinical Nursing experience
