RN Care Manager (Discharge) - PACE
About The Position
This is a hybrid position that may require some travel West and South Portland service areas. The Registered Nurse Care Manager focuses on ensuring safe and timely discharges from acute hospital settings to the community. This role is responsible for safe and timely discharge planning from acute care settings to community‑based care, with a strong focus on PACE continuity and participant‑centered care. This role functions as a member of the PACE Inpatient Management Team. Providence PACE is a Program of All-Inclusive Care for the Elderly that strives to keep older adults as healthy as possible living in the community through clinics, home visits and more. Requirements
- Education sufficient for clinical licensure
- Oregon Registered Nurse License
- 1 year Long term care or geriatric nursing
Nice To Haves
- Bachelor's Degree Nursing
- Discharge/care management experience
- Medical surgical or rehabilitation medicine
