Requirements
- Active RN license in the practicing state.
- Experience in case management, utilization review, or clinical coordination preferred.
- Strong clinical, organizational, and communication skills.
- BLS required; CCM or specialty case management certification preferred.
Responsibilities
- Assess patient needs and develop individualized care plans.
- Review treatment plans, hospital stays, and services for medical necessity.
- Collaborate with providers, interdisciplinary teams, and insurance entities.
- Educate patients and families on care plans and available resources.
- Document care, reviews, and compliance with regulatory standards.
