About The Position

Optimize patient care transitions and resource utilization in a premier academic medical center. Lead utilization review, discharge planning, and interdisciplinary coordination for high-acuity patients while ensuring compliance with EPIC documentation standards.

Requirements

• ADN/BSN from an accredited program
• Current and active New York State Registered Nurse (RN) license is required.
• Basic Life Support (BLS) certification is required; candidates must hold a current certification or be willing to obtain recertification prior to employment.
• Minimum of 1 year of recent experience (within the last 2 years) in an acute care hospital setting of similar size and complexity.
• Proven experience in hospital case management within an acute care environment is essential.
• Proficiency in the EPIC Electronic Medical Records (EMR) system is required.

Nice To Haves

• Previous experience in Emergency Department Case Management (ED CM) is highly desirable and will be considered a strong asset.

Responsibilities

• Conduct utilization reviews for appropriate care level/coverage
• Coordinate discharge planning and post-acute care transitions
• Document care coordination in EPIC
• Ensure compliance with payer guidelines and regulations
• Collaborate with ED teams for rapid case management
• Analyze resource utilization trends and recommend improvements
