About The Position

Registered Nurse Seeking elite Registered Nurse with experience in wound care and infusions Home Health Experience highly preferred! This position is part time salary and will require dedicated availability on Saturdays & Sundays. The RN will be expected to be available in their territory to admit new patients / perform patient care on Saturdays and Sundays. Generally, this mean's the RN will be responsible for seeing 3 patients per day (e.g. 2 SOCs and a ROC or regular visit). Requirements

  • Strong organization skills and time management
  • Able to multitask and accomplish and meet timelines
  • Professional appearance and effective communication skills
  • Current Texas Registered Nurse (RN) license
  • Three years of clinical nursing experience
  • Current Texas driver's license
  • CPR Required
  • Experience with healthcare software (Homecare Homebase preferred)
  • Competency to carry out any skilled procedure that the physician orders that is within the scope of the Nurse Practice Act

Responsibilities

  • Registered Nurse is responsible for comprehensive assessments in the patient's home, including the completion of skilled documentation through Homecare Homebase.
  • Evaluate patient and home health environment and performs skilled home health assessments; clinical documentation and visit notes; care plan development; communication with physician, care team members, patient and family.
  • Comfortable treating wounds and performing infusions
  • Serve as the patient's advocate and maintain ongoing communication with physician, referral sources, caregivers and facility management to facilitate coordination of care.
  • Administer medications and treatments as prescribed by physician. Provides direct patient care including infusion therapy, wound care, teaching, disease management.
  • Provide direct supervision of nursing clinical care team members (LVN's & HHA's).
  • Participate in regular case conference and in-services
