About The Position

About the Role: Southwoods Health at Home, a division of Southwoods Health established in 2022, is looking to hire an additional RN. The RN Case Manager manages an assigned caseload within patients’ homes, overseeing the entire continuum of care from initial intake through discharge.

Requirements

Active RN license in the state of Ohio.
Current AHA BLS certification.
Valid Driver’s License and proof of current automobile insurance.
Strong critical thinking and problem-solving skills.
Proven leadership skills and the ability to delegate appropriately.
Effective communication and clinical documentation skills.
Exceptional prioritization and time management abilities.
Commitment to maintaining a professional demeanor at all times.

Nice To Haves

Bachelor of Science in Nursing (BSN) preferred.

Responsibilities

Participates as a collaborative team member to support optimal patient outcomes and experiences.
Develops and implements care plans to meet patient needs safely and effectively.
Practices cost-containment by utilizing resources appropriately.
Maintains patient privacy and ensures the confidentiality of all protected health information (PHI).
Monitors changes in patient conditions and executes ordered interventions accurately.
Documents patient status and responses to interventions with high accuracy.
Provides regular updates to the multidisciplinary team regarding progress toward goals of care.
Participates in facility training and continuous learning opportunities.
Ensures all processes within the assigned physician practice maintain compliance with regulatory agencies.
Performs other duties as assigned.
