About The Position

About the Heart Optimization & Evaluation (HOPE) Clinic Join the Heart Optimization and Evaluation (HOPE) Clinic at Corewell Health, a multidisciplinary program caring for patients with advanced heart failure. HOPE RNs play a vital role in managing a complex, high-acuity population through education, clinical triage, and close collaboration with providers and specialty teams. What You’ll Do • Triage symptoms and manage patient concerns to reduce hospitalizations. • Educate patients and families on heart failure self-management. • Administer IV diuretics and perform point-of-care testing. • Collaborate with physicians, APPs, pharmacists, medical social workers, and device specialists. • Support patients using advanced cardiac devices and AI-enabled monitoring tools. HOPE is open Monday through Friday, closed weekends and holidays. There is a Rotating Work-at-Home opportunity provided staffing in the clinic is optimal. Scope of Work As a Nurse with Corewell Health, providing top-notch patient care is paramount. You'll use your critical thinking skills and the nursing process to assess, diagnose, plan, implement, and evaluate patient care. You'll be responsible for upholding the standards of nursing excellence as described by the ANA, including the code of ethics. You'll be a vital member of our team and strive to achieve the best possible outcomes, making a real difference in the lives of our patients and their families.

Requirements

  • Associate's Degree Nursing
  • LIC-Registered Nurse (RN) - STATE_MI State of Michigan Upon Hire required
  • CRT-Basic Life Support (BLS) - AHA American Heart Association 90 Days required Or CRT-Basic Life Support (BLS) - ARC American Red Cross 90 Days required
  • CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association WITHIN 90 DAYS Cardiovascular Advanced Heart Failure Ambulatory, Big Rapids Cardiac/Pulmonary Rehab, Big Rapids Pre-Op Call, United Cardiac Rehab, Radiology, Interventional Radiology, Pediatric Cardiology - Lansing 90 Days required

Nice To Haves

  • Ideal candidate will be an RN possessing 2+ years of Cardiology experience, especially in ICU or Heart Failure; may consider Cath Lab, ED, and other Cardio experience
  • We are hoping to hire 2 RNs; one that would hopefully like to eventually be a Charge RN
  • Schedules may be 8, 9, or 10-Hour Shifts and schedules may have to be scattered
  • These are mostly On-Site positions with the possibility of some Remote work, based on staffing levels

Responsibilities

  • Triage symptoms and manage patient concerns to reduce hospitalizations.
  • Educate patients and families on heart failure self-management.
  • Administer IV diuretics and perform point-of-care testing.
  • Collaborate with physicians, APPs, pharmacists, medical social workers, and device specialists.
  • Support patients using advanced cardiac devices and AI-enabled monitoring tools.
  • Assesses and monitors the overall needs of patients / family.
  • Documents nursing care / teaching consistent with documentation guidelines.
  • Ensures that required documentation for activities within areas of responsibility is performed in an accurate and timely manner.
  • Develops / maintains an individualized patient plan of care with the patient and consistent with patient / family changing needs / goals, reflecting the full scope of practice.
  • Delivers care in a manner that is culturally sensitive and preserves patient autonomy, dignity and rights.
  • Articulates the professional nursing role to the patient, family and health team members.
  • Communicates with other members of the health care team, including triage of incoming calls, in order to ensure continuity and coordination of care.
  • Acts as a patient / family advocate.
  • Initiates patient discharge planning at time of patient contact and facilitates a smooth transition to the next point of service / home.
  • Identifies, facilitates, and evaluates outcomes of nursing care for an individual patient or group of patients.
  • Evaluates patients' and family educational needs; develops and implements appropriate teaching strategies or makes appropriate referrals.
  • Provides evidence-based nursing care consistent with the plan of care, guidelines of care, physician orders, and in accordance with health system policies, procedures, and ANA / regulatory standards.
  • Demonstrates a high standard of moral and ethical behavior.
  • Formulates decisions and actions based on ethical principles, using the ANA Code of Ethics for Nurses to guide practice.
  • Advocates for privacy, confidentiality and security of patient, staff and organization data.
  • Assumes an autonomous leadership role and delegates appropriately to support staff to facilitate safe, quality patient care in a cost-effective manner.
  • Participates in ongoing process improvement activities through the application of clinical paths, QI processes, and other related activities.
