About The Position

Field-Based Clinical Leadership Role Position Overview Abernathy Home Care is seeking an experienced Registered Nurse (RN) to serve as a Home Care Nurse Manager to support our growing home care programs serving older adults in the community. This is not a desk-only position. The Nurse Manager plays a key leadership role in the field, providing clinical oversight, caregiver supervision, client assessments, and support to our nursing and caregiver teams. The Nurse Manager will work closely with our Director of Nursing and operations team to ensure high-quality care delivery, regulatory compliance, and strong support for both clients and caregivers. This position requires a nurse who is organized, proactive, comfortable in the field, and experienced in home care services. Schedule Full Time Monday – Friday 8:00 AM – 4:00 PM Occasional flexibility may be required for urgent clinical situations.

Requirements

  • Active Massachusetts Registered Nurse (RN) license
  • Minimum 2–3 years nursing experience
  • Home care experience required
  • Strong communication and leadership skills
  • Ability to work independently in the field
  • Reliable transportation for client visits
  • Comfortable using electronic documentation systems

Nice To Haves

  • ASAP / elder services program experience preferred
  • Spanish speaking preferred

Responsibilities

  • Clinical Oversight
  • Conduct initial client assessments and reassessments in client homes
  • Monitor changes in client conditions and coordinate care adjustments
  • Ensure care plans are appropriate, updated, and being followed by field staff
  • Provide clinical recommendations to support safe care in the home Field Staff Supervision
  • Conduct supervisory visits for home health aides, homemakers, and personal care staff
  • Provide guidance, coaching, and support to caregivers in the field
  • Conduct caregiver competency evaluations and field observations
  • Address and assist with resolving client or caregiver concerns Nursing Oversight
  • Provide oversight and support for per diem nurses conducting visits and assessments
  • Participate in training and onboarding of nursing staff as needed
  • Provide clinical consultation to staff regarding client care needs Client Care Management
  • Conduct service start visits and clinical assessments for new clients
  • Participate in care planning and coordination with families and referral sources
  • Assist with addressing service concerns or quality issues when they arise Compliance & Documentation
  • Maintain accurate documentation of all visits and assessments
  • Ensure documentation meets home care regulatory and agency standards
  • Assist with quality assurance and internal clinical reviews
  • Support compliance with state-funded home care program requirements Collaboration
  • Work closely with scheduling, care coordination, and operations teams
  • Participate in internal case discussions and care planning meetings
  • Support agency leadership in maintaining high standards of care delivery
