About The Position

Join Our Team as an RN Home Health Nurse with a Sign-On Bonus! Phoenix Home Care & Hospice is dedicated to delivering exceptional home health care and hospice services. Location: Kansas City, MO and surrounding areas. Available Shifts: PRN Sign-On Bonus: $5,000

Requirements

• Current RN Nursing License in the state of practice.
• Previous home health experience preferred.
• Ability to work effectively in a fast-paced environment.
• Excellent organizational and time-management skills.
• Compassionate attitude with a focus on customer service.
• Valid driver's license and access to reliable transportation.

Nice To Haves

• Previous home health experience preferred.

Responsibilities

• Conduct skilled nursing visits for patients in their homes.
• Engage in direct patient interactions to assess health conditions.
• Educate patients and families about health management, including CHF, COPD, diabetes, etc.
• Provide wound care and medication management.
• Document patient conditions and report significant changes to the physician.
• Collaborate with a team of other healthcare professionals, including Physical Therapists and Occupational Therapists.
