RN Admissions Nurse – Hospice (Lakeland, FL) Compassionate Care Hospice, an Amedisys Company This position is based in Lakeland, FL and requires daily travel within Polk County. Schedule: Monday-Friday, 9:00 AM - 6:00 / 10:00 AM - 7:00 PM (flex). To participate, you must be enrolled in an Amedisys medical plan. Orientation, Onboarding & Training You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience.

Requirements

  • Current RN license, specific to the state(s) you are assigned to work.
  • 2+ years of clinical RN experience in acute healthcare setting(s)
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
  • Hospice or home health experience

  • Comfortable with daily travel between facilities
  • Proficient in electronic medical record (EMR) systems, HCHB exp.

Responsibilities

  • Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocial and spiritual needs.
  • Coordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team.
  • Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physician.
  • Functions as a liaison between disciplines and facilities during the admission process.
  • Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate.
  • Communicates significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team. Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as required.
  • Performs on-call responsibilities and on-call services to patients/families as assigned.
  • Performs other duties as assigned.
