RN I • ASN RN II • 6 years or more of RN experience with an ASN/ADN degree OR, • 3 years or more of RN experience with a Bachelor of Science in Nursing (BSN) • BSN RN III • 12 years or more of RN experience with an ASN/ADN OR, • 6 years or more of RN experience with a BSN OR, • Masters/Doctorate of Nursing The RN Hospital Patient Navigator assists patients and families regarding plan of care, insurance, etc. This position works with patients and families to identify barriers in patient care and how to remove the barriers. The Hospital Patient Navigator ensures patient care is being handled appropriately and patient care plans are being followed.

Requirements

  • Registered Nurse | North Dakota Board of Nursing (NDBON) or Compact State Agreement | Prior to Start Date | HR Primary Sources
  • Basic Life Support (BLS) | American Heart Association (AHA)| Within 90 days of Start Date | Learning Management System
  • Temporary RN license accepted prior to start.
  • Must hold an unencumbered nursing license prior to start.
  • This position requires proficiency in reading, writing, and speaking English to ensure effective communication in the workplace and with patients, families, and team members.

Responsibilities

  • Initiates contact with new patients and arranges follow-up contact as appropriate for patient and family needs.
  • Ensures that patient psychosocial assessment is completed and identified needs are addressed with appropriate interventions and follow-up arranged.
  • Provides patient and/or patient’s family with appropriate information regarding the plan of care.
  • Acts as a resource for insurance and reimbursement needs.
  • Participates in the patient’s care through collaboration with physician and caregivers.
  • Communicates with referring facilities, or agencies including hospitals for coordination of care and relay of information, as needed.
  • Works closely with staff to evaluate data and identify barriers patients experience to care.
  • Assists with integration of identified relevant barriers into Quality Improvement Plan.
  • Communicates with medical staff, coordination team and nursing staff regarding appropriateness of admission and/or need for continued stay along with the discharge plan.
  • Manage patient flow to ensure that patient care plans are being followed and that the appropriate services are administered at the appropriate times.
  • Collaborates in establishing and maintaining patient navigation program goals, objectives and evaluating progress toward their attainment.
  • Performs other duties as assigned or needed to meet the needs of the department/organization.
