About The Position

We help each patient achieve the best possible health through every stage of life. We do this by providing an accessible, innovative healthcare experience that is built around our patient’s needs. We are seeking a full-time experienced RN or LPN to join our nursing team. This is an in-person triage position in Bedford, NH. You will be part of a collaborative work environment that includes social workers, care managers, and another triage nurse in the same location. Hours are Monday – Friday plus rotating Saturday hours. The successful candidate will ensure that communication with each patient caller is handled professionally, compassionately, and accurately. When appropriate, the caller’s symptoms will be assessed and triaged according to established DMC nursing processes and approved guidelines to determine the appropriate level of care and/or self-care advice. In addition, the Triage Nurse will support the nursing team by assisting in management of patient messages and communicating necessary information to both patients and providers. As a DMC Triage Nurse, you will also assist with transitions of care activities, including contacting patients after hospital discharge, reviewing discharge instructions, performing medication reconciliation, and updating electronic medical records.

Requirements

  • Completion of an accredited RN or LPN program (required).
  • NH State licensure as a Registered Nurse or Licensed Practical Nurse (required).
  • BLS Certification (required).

Nice To Haves

  • 1-3 years of outpatient primary care, or triage nursing experience (preferred)
