As a Care Coordinator (RN), you will collaborate with multidisciplinary caregivers across the continuum of care to provide comprehensive care coordination and disease management for patients with chronic conditions, with a focus on IBD and UC. In this role, you will monitor hospital inpatients, ensure timely follow-up visits, and manage the completion of tests, procedures, imaging, and genetic results to support providers in delivering optimal treatment. You will track patient adherence to treatment plans and required follow-up, triage patient concerns for timely resolution, and facilitate provider involvement when needed. This role supports high-acuity patients who require the specialized skills of a trained RN, particularly in complex motility cases, ensuring meticulous care and thorough review of disease processes. Additionally, you will assist with procedures in the Motility Lab and work collaboratively with providers in the Gastroenterology and Colorectal Surgery departments to triage patient concerns and direct care effectively, contributing to the growth of our subspecialty groups and the delivery of timely, high-quality care. A caregiver in this role works days from 8:00 a.m. to 5:00 p.m. or 7:30 a.m. – 4:30 p.m.
