Responsible for the coordination of care, patient assessment, patient education, triage, and various other interventions. Requires excellent time management skills and ability to prioritize/triage the needs of the patients and the practice. Occasionally works in emergent patient care situations requiring critical thinking and prompt assessment skills. Responsible for utilizing the professional nursing process to provide individualized nursing care to patients in an ambulatory setting. Cares for patients in all phases of specific states including assessment, disease maintenance, treatment, and follow-up as patients move in both inpatient and outpatient setting. Must be skilled in prioritizing communication skills both with patients and interdisciplinary care team. Patients triaged symptoms will include post-surgical, active oncology treatment symptom management, or post-treatment long-term symptoms. Collaborates with providers and assists with procedures when completed in clinic. Will travel to outreach clinic site with physician at least once a month (no overnight stay). Must have working knowledge of electronic medical records. Abides by respective healthcare agency's policies. Provides a quality of service that customers describe as excellent.

Requirements

  • Graduate from an accredited nursing program required, including, but not limited to, American Association of Colleges of Nursing (AACN), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
  • Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
  • Obtains and subsequently maintains required department specific licensure, competencies and certifications.
  • Must have working knowledge of electronic medical records.
