About The Position

We are seeking a Nurse Manager to oversee and lead our Transitional Care Unit housed within our beautiful Rehabilitation Hospital on Ward Hill in Haverhill Ma. Essential Duties: To provide exceptional care to all residents including but not limited to: -Under the direction of the Director of Nursing Services, holds 24-hour authority, responsibility and accountability for the clinical management of assigned resident/patient care unit. - Obtain report from nurse being relieved and records sufficient information to implement appropriate follow up action as necessary - Provides report to nurse coming on duty, including sufficient information for follow-up action if necessary - Conducts resident rounds, reporting problems to nursing supervisor and initiating corrective action - Administers and documents medication, enteral nutrition and treatment per physician order and accurately records all care provided and observations daily - Performs comprehensive assessments of each resident on unit - Develops care plan based on each resident's assessment - Obtains physician orders and any revisions for all treatment and medications needed - Assesses on a weekly basis the skin condition, stage, size, site, depth, color, drainage and odor of condition - May conduct staff meetings; coordinate services rendered by nursing personnel with those rendered by personnel of other departments. - Observes infection control policies and procedures relating to nursing services. -Directs and supervises activities of professional and non-professional nursing personnel - Supports and participates in In-services Education Programs. - Renders immediate assistance to any resident in need - Provides clinical supervision of nursing assistants providing direct patient care - Assumes responsibility for compliance with federal, state and local regulations within the assigned unit - Counsels nursing assistants and communicates clearly with Director of Nursing if issues arise - Collaborates with interdisciplinary team to assure that high quality resident care is maintained - Prepares nurse assistant assignments and explains all pertinent resident care - Supervises distribution of nourishments - Supervises the removal of all postural supports - Orders, receives and documents medication according to facility procedure - Supervises response to residents calls for assistance and responds to call lights as necessary - Insures that all physician renewal orders and physician documentation is timely and appropriate

Requirements

  • Graduate of an accredited Registered Nurse school of nursing
  • Current and active RN license in the state of Massachusetts
  • Current certification in Basic Cardiac Life Support in accordance with AHS or ARC standards
  • Minimum of three (3) years recent clinical experience preferably in Gerontological or Rehabilitative Nursing.

Nice To Haves

  • Prior MDS Management experience preferred.
