RN Wound Care Home Health
About The Position
Performs professional nursing services requiring the application of the biological, physical and social science and nursing skills in the care of the sick, the prevention of disease, and the conservation of health. Works in an interdisciplinary relationship with other members of the Corporation's staff to provide coordinated Home Health services to individuals and their families in accordance with a Plan of Care established with the medical provider, patient and family. Performs nursing assessment, planning, care and evaluation of complex wounds, ostomies, and continence-related conditions in accordance with the established Plan of Care. Assumes primary case management functions for assigned patients.
Requirements
- Completes and maintains all competencies and trainings as required for role.
- Graduate of an accredited school of Nursing
- Relevant Experience
- Current Valid Driver's License
- PA Registered Nurse License
- BLS Certification (within 60 days of hire)
- Wound Care specialty certification or Certification in Wound, Ostomy and Continence
