Responsibilities: AWARE’s SDMI Referral Coordinator is a person who has the responsibility of coordinating and facilitating referrals for the SDMI Waiver. Review and upload documents regarding clinical and financial eligibility for SDMI Waiver to the member's chart. Manage SDMI Waiver Waitlist to include documenting referral information, receiving communications regarding waitlist information, and maintaining quarterly contact with waitlist members. Upload annual Level of Impairment forms provided by Mountain Pacific to member record. Follows State of Montana and A.W.A.R.E., Inc. rules, policies, procedures, and guidelines as they apply to this service. Must demonstrate—on a consistent basis—the willingness and ability to productively utilize work time in order to meet the needs of the individuals being served as directed, with the understanding that specific fiscal goals must be met monthly. Record referrals, and other data entry assignments via AWARE’s Electronic Healthcare Record System. Engage in follow up, assessment of need, and referrals to SDMI Waiver services. Coordinate with Clinical Administrator to support SDMI Waiver enrollment process. Engage in routine communication with SDMI Waiver providers regarding service availability.
