The Senior Director, Practice Transformation is a critical leadership role sitting directly within the Transformation function. Reporting to the Head of Transformation, this leader is responsible for ensuring clients are fully adopting and embedding our products into their workflows - creating the conditions for sustained utilization and account expansion. This role owns the full adoption lifecycle across hospital systems, group practices, clinics, and individual physician offices, and leads a team of clinical field nurses and directors who execute in the field. This is not a coordination or implementation role. Adoption drives everything — without it, there is no expansion, no retention, and no transformation. The right candidate understands that and operates accordingly: independently diagnosing why adoption is stalling, building a targeted strategy to break through, and driving execution across both their field team and the client organization. Deep fluency in provider workflows, reimbursement dynamics, and the behavioral levers that move physicians and clinical staff is non-negotiable. A Day in the Life Adoption Strategy & Account Planning Develop and own a structured, account-by-account adoption strategy across the portfolio of hospital systems, group practices, and individual practices Conduct independent account diagnostics — identify workflow barriers, stakeholder resistance, reimbursement misalignments, or training gaps driving underutilization Translate product features into concrete clinical and operational value propositions tailored to each practice setting and specialty Build deployment and rollout plans that account for enterprise complexity, including multi-site group practices and integrated health systems Clinical Workflow & Practice Integration Lead the strategic embedding of the product within practice workflows — not just training, but genuine workflow redesign where needed Map and leverage the patient care journey across both primary care and specialist environments to identify the highest-value integration points Advise practices on FFS-to-VBC transitions and position product use within population health and value-based care frameworks Demonstrate fluency in single and multi-specialty group practice dynamics, including clinical hierarchies, referral patterns, and operational constraints Team Leadership & Development Lead, develop, and hold accountable a team of Directors and clinical field nurses driving in-practice adoption across the account base Establish clear performance expectations, structured account ownership, and data-driven prioritization for the field team Coach Directors from tactical account management to genuine strategic ownership - and hold them accountable for building the same capability in their nurses Build a team culture of ownership, accountability, and continuous improvement Stakeholder Engagement & Executive Presence Navigate complex stakeholder environments — engaging credibly at the C-suite and CMO level while also influencing front-line staff and physicians Build and maintain durable relationships with practice administrators, medical directors, and clinical champions across the account base Represent the organization in strategic conversations with health system leadership around population health strategy, care transformation, and technology integration Reimbursement & Value-Based Care Advisory Advise provider accounts on billing and reimbursement implications of product adoption, with working knowledge of CPT coding, payer policy, and quality incentive programs Fluent in VBC frameworks including ACO structures, MSSP, HEDIS metrics, and shared savings models — able to position the product as an enabler of VBC performance for both PCPs and specialists Identify reimbursement-driven adoption opportunities and work with internal stakeholders to develop supporting materials
