About The Position

The Senior Resident Services Coordinator will provide a vital role in engaging supportive housing residents and providing services to help them retain housing, develop and achieve attainable housing-focused goals. The Senior Resident Services Coordinator also supports residents at Valencia Gardens (VG) utilizing a trauma informed approach to help improve their daily lives. In addition, the role of the Senior Resident Services Coordinator will be to facilitate linkage to community resources, research and secure additional partnerships to provide services onsite offerings and 3rd party contracted partners for residents as needed.

Requirements

  • Bachelor’s degree in related field or 3 years of experience in community services coordination and social service.
  • Excellent interpersonal skills, ability to work effectively with people of diverse backgrounds.
  • Experience working with and serving homeless populations, diverse populations, low to moderate income families, people with mental health issues, substance abuse, disabilities, legal issues and older adults.

  • Knowledge of Federal, State and City benefits programs and local services in the Mission District and San Francisco.
  • Ability to maintain confidentiality of sensitive information is a must.
  • Ability to follow through and complete job duties when assigned in a timely manner.

Responsibilities

  • Responsible for the Resident Services Operations at Valencia Gardens.
  • Responsible for managing the reservations of the VG Community Room which includes the following: working with internal and external agencies in reserving the room, managing dates, times, room setup, completing liability and room reservation forms, collecting fees, and working in accordance to property management.
  • Responsible for the oversight of the VG Computer Lab, working collaboratively with our community computer partner in making sure that the computer lab functions efficiently to serve the Residents of VG.
  • Establish a team environment and work collaboratively with the Service Coordinator and VG Property Management.
  • Performs other duties as considered appropriate and consistent with the mission of Mission Housing, including working at occasional community-building activities, programs and events (i.e., annual on-property community activities, Carnaval, Sunday Streets, fundraisers, etc.) – as needed or assigned, during or outside regular business hours.
  • Identifies the strengths of residents for asset-building and personal development as well as their needs for supportive services.
  • Researches, identifies, and links residents with resources such as rental assistance, transportation, adult education, childcare, health services, and employment assistance. SSI, Medicare, food stamps, and prescription drug assistance, senior services and families for all pertinent services that would benefit our community/residents.
  • Establishes relationships, maintains and also follows-up with community service providers.
  • Works closely with Property Management staff to identify specific supportive needs as related to lease violations and household stability - in addition with following up and updating supervisors on the outcome of any referrals provided to the Senior Resident Services Coordinator.
  • Compiles information and eligibility requirements for neighborhood and city-wide resources.
  • Organizes community resource displays with information and applications.
  • Responsible for coordinating and developing tools for service coordinators and residents ex: Welcome Packet, Registrations for Programs etc.
  • Conducts comprehensive screenings and assessments with individuals (or families) to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information, as appropriate; conducts intakes and ensure that appropriate follow-up and/or referrals are complete and documented.
  • Delivers onsite programs, ensures implementation of programs at assigned locations to meet agreed internal standards and objectives and applicable regulatory expectations; provides activities for community building and empowerment.
  • Assesses barriers facing the clients and develops a case plan; creates and tracks service plans; coordinates individualized planning with clients to meet short- and long-term needs.
  • Provides service linkages and support systems to ensure identified needs are met.
  • Coordinates case conferences with residents, consults with service team, monitors the progress of clients on case plan, assists clients to meet the agreed upon goals.
  • Provides ongoing creative outreach and engagement for supportive counseling, referral, and involvement in building and community events, as well as ongoing support such as: assists residents, gathering all documentation which would keep households in compliance with management/good standing; assists residents for annual rectification/appointments/following up on outstanding action items and/or any other issues regarding housing stability and housing reasonable accommodation.
  • Provides ongoing support to residents who need to be assisted by Eviction Defense Collaborative [https://evictiondefense.org/], Bay Area Legal Aid [https://baylegal.org/], Open Door Legal [https://opendoorlegal.org/], Housing Rights Committee [http://hrcsf.org/], back rent referrals and personal finance support by San Francisco Smart Money Coaching [https://sfgov.org/ofe/meet-financial-coach] or other service providers.
  • Prepares and submits all necessary program documentation by Mission Housing and external agencies as required.
  • Assists in and/or provides crisis intervention and follow-up (including but not limited to child abuse or adult abuse and domestic violence reporting), implements mandated reporting policies and procedures.
  • Provides direct case management services for identified residents who exhibit signs of functional impairment, including support to develop independent living skills; works with SFPDH to address cluttering issues as well as Child Protective Services (CPS)/ and Domestic Violence cases.
  • Prepares group and one-on-one services to residents and supports community-building activities at assigned properties, provides services using harm reduction principles.
  • Works with the residents to support independent living and personal and professional goals.
  • Assists resident in the event of any emergency such as fires or floods (e.g., American Red Cross [http://www.redcross.org]).
  • Provides crisis intervention, conflict resolution and de-escalation services.
  • Coordinates Food Box programs for VG Senior Residents.
  • Participates in team meetings, staff meetings, department meetings, networking meetings and organizational work groups.
  • Becomes knowledgeable with housing programs (SF RAD, Sec. 8 & SFHA Subsidy Programs).
  • Develops and facilitates groups and community building activities.
  • Provides back-up coverage for other Resident Services staff.
  • Performs other duties as assigned.
  • Responsible for maintaining, scheduling and booking Community Room for internal and external use.
  • Responsible for coordinating various outreach methods to all Valencia Gardens Residents.
  • Explores and provides on-site educational speakers and workshops that fulfill compliance requirements for Mission Housing Regulatory Agreements for assigned sites. This may include topics such as but not limited to: health and wellness programs, senior services, youth programs, educational classes, support groups-social events-organized tenant activities, community building activities, basic cases management-information-referral and financial literacy, computer training, employment services-job searching, and other life skills services and programs, benefit assistance, advocacy and counseling.
  • Sets up office hours at each of the buildings and advertises office hours for residents to meet with and request referrals.
  • Recruits and supervises volunteers to assist with programs.
  • Assists residents in building informal support networks with other residents, family members, and friends.
  • Creates opportunities for residents to get involved in community activities.
  • Coordinates resident training for leadership roles, such as developing and implementing beneficial programming.
  • Organizes on-site community-building events.
  • Exchanges resources with other Resident Services staff and collaborates on programs and community-building activities, as appropriate.
  • Produces a monthly calendar of events and activities.
  • Provides back-up coverage for other Resident Services staff and other programs and activities such as community food pantries, outreach campaigns, social gatherings, networking meetings, and any other kind of assistance.
  • Performs other duties as assigned by supervisors and as required by any regulatory agreements, grants and/or Memorandum of Understanding (MOU) with partners.
  • Utilizes Salesforce database to install notes and pertinent information on a daily basis.
  • Accurately completes and submits timesheets by deadlines, reporting hours of work performed and allocating time on ADP, monthly service reports, grant reports, activities budgets, program reporting, and any other required paperwork/reporting needed for the department.
  • Submits accurate monthly reconciliations to the accounting department in a timely manner.
  • Conducts surveys to assess resident needs, tabulating survey results to evaluate programs in order to plan relevant programs for the communities that you serve.
  • Creates monthly calendar of events and activities, producing outreach flyers and quarterly newsletters to keep the community informed if applicable.
  • Responsible for maintaining a current and updated Outlook calendar of activities at assigned sites.
  • Maintains thorough resident files on all work performed, follow up and documents outcomes.
  • Attends appropriate internal and external meetings as assigned by supervisor or Department Head. Such meetings may include Resident Services Department Meetings, Staff Training, Programs Supportive Housing Network meetings, Public Hearings, Community Meetings, etc.
  • Meets monthly/quarterly with the City/County Housing Services Program SF/HOPE.
  • Performs other duties as assigned.
