About The Position
Senior Doc, part of Brigade Health, is seeking a Primary Care Physician (MD/DO) to provide post-acute and long-term care services in Skilled Nursing Facilities (SNFs). This role is ideal for physicians who enjoy caring for medically complex older adults, building strong relationships with facility teams, and practicing in a collaborative, value-based care model. Requirements
- MD or DO
- Board Certified / Board Eligible in Internal Medicine, Family Medicine, or Geriatrics
- Active state medical license (or ability to obtain)

- Experience in SNF, post-acute care, geriatrics, or long-term care preferred.
Responsibilities
- Provide primary and post-acute care in Skilled Nursing Facilities
- Manage admissions, follow-ups, and chronic care for geriatric patients
