About The Position

Introduction to the Company Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients. Overview You will be working with low-income clients who reside in special housing complexes and those who are transitioning out of nursing homes. Who You Are: You are interested in working with an interdisciplinary team of health professionals helping individuals transition from a hospital to home care setting and assisting them to remain in the community. You enjoy helping clients achieve their personal goals of remaining in the community. You want to work for a company where you will be valued for your skills and contributions to patient care

Requirements

  • Degree (Bachelors required) in Social Work or social science or related field such as gerontology, health services, drug or alcohol/mental health training.
  • Valid California Driver’s License and eligible to be insured under our liability policy with a clean DMV report
  • Your own vehicle and proof of current auto insurance
  • CPR certificate required within 30 days of hire (employer sponsored classes provided)
  • Willing to drive around Chico area, and in Butte and other surrounding counties to make home visits to clients. (Personal Protective Equipment & Training is provided)
  • Sits, stands, bends, lifts, walks, and moves intermittently during working hours
  • Able to lift to 35 pounds without assistance

  • Excellent written and verbal communication skills
  • Understands and can provide a comprehensive psychosocial assessment of a client
  • Ability to work with electronic medical health records and computer systems.
  • Willingness and skills to work with clients, hospital and nursing home discharge planners, family members and Powers of Attorney at times to ensure a smooth and transparent transition process
  • Enjoys working in an interdisciplinary work setting with Registered Nurses and clerical staff to perform transition coordination.
  • We will train you for this program.
  • Bilingual skills are a plus but not required.
