Requirements

  • Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
  • Bachelors required
  • Board Certified Social Worker, or related field (i.e. counseling) in the state of Alabama (CSW, LPCA, LCSW) within 6 months of hire

Responsibilities

  • Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
  • Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
  • Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan. Participates in daily interdisciplinary team meetings.
  • Conducts discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
  • Trains new staff, students, and interns.
  • Documents discharge plans, actions, interactions, communication with patients, family, caregivers, etc. regarding patient care, education, outcomes, etc. as needed in EMR.
  • Sets up/orders new dialysis, wound vacs, any DME needed through portals to facilitate transition of care.
  • Places referrals to post discharge agencies per patient/designated caregiver/legal next of kin/choice and follows through process to ensure smooth transition of care
  • Performs other duties as assigned or delegated by the department director.
